Position Overview:
This position is responsible for supporting the Elizabeth Taylor 50-Plus Network, a program for gay , bisexual, and trans (GBT) men over the age of 50. 50-Plus members include long-term survivors of the AIDS Epidemic, People Living with HIV (PLWH), and older GBT individuals impacted by HIV, regardless of HIV status. The Health Educator provides individualized support, group programming, and outreach. This role partners with the Community Engagement and Health Navigation teams to ensure that 50-Plus members are connected and engaged in social supports, health care, and social services. 50-Plus offers group-based and individual support; peer-based support; and cultural events, community forums, socials, and volunteer opportunities that increase a sense of community belonging.

Essential Duties and Responsibilities:

Health Education & Individualized Support:
Provide support and resources to 50-Plus members through individual sessions. Responsibilities include building ongoing relationships, providing referral and linkage to substance use, health, and social services. Use motivational interviewing practices with a focus on self-care, reducing isolation, harm reduction, and health improvement. Provide individualized support on issues related to HIV and aging, including testing and treatment, medication adherence, PrEP and nPEP, and prevention of HIV and related health disparities. Assists members in contemplating change, making goals, and plans to meet them. Support may include medical accompaniment, providing transportation assistance to and from medical care visits, and assisting members navigate health care provider communication.

Program Design & Facilitation: Design and facilitate workshops for 50-Plus on issues related to health and wellness, HIV & Aging, and other topics identified by members. Includes coordinating groups, events, and activities.

Outreach & Engagement: Build relationships with individuals over the age of 50 impacted by HIV and the AIDS epidemic. Conduct outreach at events and engage potential members. Develop strategies and practices to assess community needs and ensure needs are reflected in programming and model of care.

Collaboration: Promote collaboration and referrals between Elizabeth Taylor 50-Plus Network and Black Brothers Esteem, a program for African-American men who GBT men; D.R.E.A.A.M. Project for young African-American GBT men; TransLife, a program for transgender & gender non-binary folks; and Latino Programs. Ensure collaboration between the foundation’s three locations: Mid-Market, Strut, and 6th Street Harm Reduction Center.

Documentation: Document individual sessions and group interventions in Electronic Medical Records.

Education and Experience:
Three years’ experience working with LGBTQ people and/or people impacted by HIV required. Bachelor’s Degree highly regarded. Understanding of HIV/HCV disease prevention and treatment. Bilingual English and Spanish, Russian, Cantonese, or ASL preferred

Work Environment:
This position is primarily located at 1035 Market Street, San Francisco. Requires traveling to other office locations or off-site meetings, such as our Castro location, as required by the job. Requires weekend and evening working hours.
