POSITION DESCRIPTION
The Health Educator is responsible of assessing, implementing and evaluating preventive and health education program to encourage Members to take more responsibility for their health and modify behavior that will keep them healthy or that will assist them to improve their health status and healthier lifestyle.

ESSENTIAL DUTIES AND RESPONSIBILITIES
Plans and implements health educational initiatives aimed at MMM and PMC members, based on behavioral modifications and lifestyle wellness. Obtains necessary information in order to identify member’s needs and problems. Develops specific educational plans and or interventions for individual, groups and population according with Special Needs Program (SNP) population (Chronic, Dual, and Institutionalized members). Develops educational campaigns regarding the elderly population needs, HEDIS indicators compliances and/or corporative goal (ex. Support to STAR Program). Designs and delivers health education seminars of various topics to members, general community and other audience. Educate members using educational strategies appropriate for member or audience needs such as face to face education, telephonic interventions, group education, and written educational materials. Advocate, inform and educate members of SNP of health risk, care and social services (community-based) according with the corresponding needs. Offer benefits interpretation to members according with the member’s current needs. Performs literature review regarding the public health, health risk, health promotion models and others topics regarding the elderly population. Coordinates the provision of health education and preventive health activities in all MMMHC service areas including the Case Management Program, and other educational programs. Establishes relationships with pharmaceutical companies for co-sponsorship of educational activities and with outside organizations/community agencies. Collaborates in the publishing of the quarterly Member newsletter with educational articles. Identifies community resources and support groups according with HEP interventions, member’s needs and Department Goals. Assists Case Managers in developing effective educational strategies to encourage Members to modify their behavior to achieve a healthy life style. Participates in round clinics with the multidisciplinary care team, the primary care physician, and other participating personnel. Participates in home visits to members as needed. Participates on meetings, as appropriate with primary care physician (PCP), IPA and administrative persons regarding the corporative projects (Influenza prevention campaign) assigned at the Program. Participates in educational courses, training and/or in services aimed to employees offered by the company. Perform population identification, health evaluation, services coordination, individual care plan, adherence program, educational intervention regarding self-management, welcome letter, communication to physicians, educational mailing, reminders, phone call to check progress or offer coaching, community agencies referral, among others according with the Special Needs population (CSNP, DSNP, ISNP). Plans and coordinates educational activities and campaigns according with the STAR Program. Acts as a resource to offer orientation activities, media interventions, among others in the community as requested or needed by Internal Departments. Have available and monitor a Mobile Phone with email access that can be available to Department staff for consultation and/or emergencies including after-hours coverage. Obtains and maintains files of educational material to be used for member education. Prepares and submits to the Team Leader a weekly schedule of activities to be performed. Prepares and submits to the Health Education Manager a monthly report of activities performed and annual evaluation of the education program. Attends departmental meetings when required. Acts as a resource for Nurse Care Managers and other MMM Medicare y Mucho Más employees. Participates in the evaluation of departmental processes and submits recommendations. Performs activities in compliance with MMM / PMC and CMS requirements. Adhere to the department policies related to care of the plan’s SNP members. Attend all required trainings related to the MOC especially in areas of care management, ICP, ICT and health assessments. Notify supervisors of any processes not in compliance with Plan’s SNP policies and MOC requirements. Clinical staff who do not complete the required annual trainings may not perform their clinical duties and care management functions until the training is completed. Bachelor’s degree in Community Health Education The jobholder must have a current unrestricted Puerto Rico Health Educator license. Requires at least 2 years of experience in planning, designing, and implementing health education programs, seminars to the older adult population. Must have knowledge in educational strategies oriented to older adult population. Must have knowledge regarding Medicare coverage and benefits, as well as CMS/ASES regulation, among others. Must have knowledge regarding NCQA standards and regulations. Must have knowledge of local community resources. Master degree in Public Health Education from an accredited university preferred.

LANGUAGE SKILLS licensed Fully Bilingual Ability to read, analyzes, and interprets instructions in Spanish and English language. Ability to analyze and understand procedure manuals, professional journals, technical procedures, or governmental regulations (i.e. CMS regulations). Ability to write reports, professional correspondence, and educational materials. Ability to effectively present information and respond to questions from groups of managers, clients, customers, and the general public.

MATHEMATICAL SKILLS Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
