The Bilingual Wraparound Facilitator/Quality Assurance Coordinator engages high-need, multi-system involved youth and their families in the High Fidelity Wraparound planning process and supports youth, their families, and their teams (including professionals and natural supports) in developing, implementing, and monitoring an integrated, individualized plan of care. Some of these teams will be facilitated in both English and Spanish. The Coordinator also approves services for youth and their families, monitors the quality of those services, and holds providers accountable and acts as a liaison between providers, youth and families, and their teams.
Several projects and goals of the Coordinator will be to:
- Increase Boulder County's ability to provide High Fidelity Wraparound services to families in need, especially for those with monolingual Spanish speaking supports that are influential in their lives;
- Assist in training and empowering team members and system partners in culturally competent service delivery;
- Be a part of a Team working to improve supportive collaboration and coordination of services by providing High Fidelity Wraparound and ensuring the quality of contracted services within IMPACT.
The Bilingual Wraparound Facilitator/Quality Assurance Coordinator is a full-time, benefited position with Boulder County. This position will generally work Monday through Friday, from 8:00 am - 4:30 pm and will require some evening work. Total hours worked will be 40 per week . This position will work out of Boulder, CO. Under FLSA guidelines, this position is exempt .
Hiring Range : $3,939 - $4,806/monthly - $47.268 - $57.672/annually
Full Salary Range : $3,939 - $5,673/monthly - $47,268.00 - $68,076.00/annually
EXAMPLES OF DUTIES
The Bilingual Wraparound Facilitator/Quality Assurance Coordinator will manage the following tasks:
- Coordinate and facilitate Wraparound team meetings made up of youth, their families, professionals, and natural supports;
- Develop integrated, individualized plans of care with teams that are based on the identified strengths and needs of the youth and family and reflect the best possible fit with their culture, values, and beliefs;
- Facilitate collaboration and create joint ownership across agencies of the integrated plan of care;
- Ensure that the Wraparound processes is delivered with fidelity to the model using the phases and activities of Wraparound;
- Assist families in identifying, recruiting, and engaging natural supports;
- Interview youth, their families, professionals, and natural supports to uncover strengths, needs, family culture, and vision;
- Create a comprehensive crisis prevention plan with the team which includes creative strategies to assist the youth to remain safely in the community;
- Build upon the strengths of youth, their families, and team members and support the development of the self-efficacy;
- Maintain knowledge of services and supports available within the community;
- Assist the youth and family in accessing the services, supports, and resources required to meet their needs;
- Approve services provided to youth and families to ensure that the most appropriate services are matched; monitor length of service;
- Act as a liaison between providers and team members;
- Monitor providers to ensure that services are adequately provided for each youth in accordance with federal, state, and county policies and standards of care;
- In partnership with the Contracts Teams, hold providers accountable by conducting reviews and site visits to monitor service delivery and contract compliance;
- Identify and track problems with providers to ensure improvement or resolution;
- Track teams' achievement toward identified goals so that progress toward less restrictive and intensive services can be accomplished while strengthening the family's ability to function;
- Demonstrate the skills required for Wraparound certification and use various types of data to improve practice;
- Complete all the necessary documentation in a strength-based manner;
- Perform all data collection required to support program delivery, implementation, and evaluation;
- Develop and implement procedures to enhance and increase internal and external efficiency and customer care; develop and train staff in utilizing documentation tools;
- Make recommendations regarding program and division funding, budget, processes, procedures, and programs;
- Participate in various committees, community programs, and boards related to program activities and responsibilities;
- Educate youth, families, professionals, natural supports, and the community about Wraparound;
- Perform other duties as assigned.

- A bachelor's degree in public administration or human behavioral science.
- Two (2) years of professional case management experience or two (2) years of direct clinical experience required.
- Ability to speak, listen, read, and write in both Spanish and English is required.
- Applicants must have a valid driver's license, a good driving record, and current automobile insurance.
- Applicants must have their own, reliable vehicle.
- A job offer is contingent on passing a criminal background investigation.
- Commitment to the Wraparound principles: family voice and choice, team based, natural supports, collaboration, community based, culturally competent, individualized, strengths based, unconditional and outcomes based;
- Strength in creating effective relationships with individuals of different races, ethnicities, religions, sexual orientations, gender identities and expressions, and other cultural beliefs and lifestyles;
- Ability to multi-task, keep track of details, and stay organized;
- Ability to listen empathetically and curiously in both English and Spanish;
- Written communication skills in both English and Spanish;
- Flexible and creative problem solving skills;
- Working knowledge of the theories, principles and concepts of social casework practice and clinical practice;
- Knowledge of community resources and agency rules and procedures related to assigned program area;
- Ability to write reports concisely;
- Ability to work effectively with other employees, clients, agencies, and the public in a collaborative manner;
- Must be able to work flexible schedule to meet the needs of families including some evenings and occasional weekends;
- Required to work in clients' homes and community environments, as necessary.
- A master's degree in public administration or human behavioral science;
- Clinical licensure;
- At least two years of experience working with high need children and youth, preferably with youth involved in the juvenile justice system, mental health system, child welfare, and/or special education or truancy;
- At least two years of experience facilitating small groups;
- Experience in utilization management.
