Serves as a liaison with residents, physicians, nurses, insurance providers and therapy department. Organizes and coordinates chart information, interdepartmental communications. Provides organizational and clerical support to health care team.

ESSENTIAL DUTIES AND RESPONSIBILITIES:
Prepares and processes admission charts, medical records, physician orders, transfers and discharges for residents. Initiates interdisciplinary communications with licensed nurse, physical, occupational and speech therapies, dietary services, pharmacy and other ancillary services for patient referrals, physician orders, transfers and discharges as required for review and signatures. Files in the floor medical record chart for the duration of residency. Organizes monthly recapitulation of physician orders, and associated paperwork for review by nursing staff and prepares to send for physician signature. Electronically prepare and transcribe orders for admissions and medication administration. Liaison to family members, physicians, Home Health and DME vendors. Telephone back-up to for Skilled Nursing staff. Schedules health care appointments for residents as well as arranging transportation and advocates for residents as needed. Carries out all aspects of the job using good judgment and problem-solving skills. Supports the healthcare team by anticipating and responding to team requests and resident needs. Communicates effectively.
