The TCC will function as the key patient advocate as it relates to coordination of AHN community healthcare services within Allegheny Health Network owned hospitals and all members of the home health, hospice, HME and infusion operational teams. Will attract new and retain existing referral sources within assigned hospital(s). PRIMARY DUTIES AND RESPONSIBILITIES: Demonstrated competency with all required products and services of AHN Healthcare@Home as outlined in each AHN hospital TCC competency checklist.
  • Assist with coordination of home health care, hospice, infusion (including pre-authorization as required for infusion), and home medical equipment (HME) services within assigned hospital(s) for appropriate admission, updating, and communication of any changes to requested services or products in conjunction with intake customer service center.
  • Follow up on patients that are hospitalized during their home health or hospice episode to assure continuity of readmission to AHN’s services.
  • Participation in case conferences as requested.
  • Promote the growth of AHN Healthcare@Home with positive public relations and excellent customer service skills.
  • Demonstrated interest, commitment and ability in liaison skills and assessment.
  • Preparation and presentation of materials in a group setting.
  • Maintain accurate and timely records of activity through ongoing marketing/activity occurrence documentation and scheduling in CPA and CIBS. Marketing/activity occurrence documentation shall be documented within 24 hours, and schedule for upcoming week is to be submitted by 4 pm on Friday of previous week.
  • Accountability and Productivity based on existing accounts, with source base changing as new accounts are established. Accounts must be maintained accurately and contain up to date information.
  • Adherence to personal or team monthly and quarterly activity, quality and customer service goals.
  • Follow best practices as outlined in policy and procedure book.
  • Maintain a high level of competency, growth and development.
  • Attend Transitional Care Services team meetings, educational in-services and yearly competency requirements.
  • Responsible to strictly adhere to mandated federal, state, local regulatory and statutory requirements as well as agency compliance policies and procedures for referral processing.
  • A focus should be on obtaining accurate and complete Patient Healthcare Information (PHI) within HIPAA minimum necessary limitations in order to appropriately coordinate AHN Healthcare@Home services.
  • Work closely with intake customer service team to manage referral intake process.
  • Follow guidelines as indicated in employee handbook.
  • Preceptor duties as necessary for newly hired TCCs.
  • Thorough knowledge of and strict adherence to Celtic’s and AHN Healthcare@Home’s Mission, Vision, and Core Values.
