We need you to bring your professional knowledge and skills to advocate for disabled or vulnerable adults. Your success will be measured through gathering objective, timely and thorough details, and assisting individuals with services that maintain their safety and dignity.

This position will be in the Behavioral Support Unit. The case managers will assess clients coming from Western State Hospital, Department of Corrections, Special Commitment Center, Development Disability Services (DOD) and Mental Health Facilities. You will work with clients in the Expanded Community Services (ECS) program. Complete other cases/assignments as deemed necessary.

This is at the specialist level and independently provides intensive and expert services to a population of high risk, dysfunctional and frail persons who are medically and financially dependent and in need of the full range of HCS services. This population includes clients who are at high risk of being placed in an institution

Some of what you'll do:
  • Conduct comprehensive assessments and reassessments at client's residence, using a laptop-computerized system to determine clients nursing level of care.
  • Develops initial and ongoing care plans, which provides the support necessary to allow clients to remain in or return to the community from nursing facilities, hospitals, or other institutional care settings: or permits an alternative to nursing home placement through MPCS. COPES and other HCS programs.
  • Provides case management services including development of a service plan. implementation of that plan and service plan review and termination.
  • Provides client advocacy, as well as consultation. networking, resource development. service delivery coordination with community partners. family support and crisis intervention.
  • Interprets Comprehensive Assessments and personal client information in view of client's ability, life style and home size in order to authorize the appropriate HCS services.
  • Instructs and educates other professionals and the public about available services as well as possible limitations of those services.
  • Makes referrals to other community resource agencies as necessary.

What we are looking for
  • Knowledge of Medicaid terminology, regulations and policies a plus.
  • Knowledge of Long Term Care programs and policy.
  • Knowledge of DSHS database systems (TIVA, CARE, Barcode and ACES online).
  • Good organizational skills.
  • Ability to handle intense and high pressure situations.
  • Ability to manage high referral intakes.

Who should apply?
Professionals who have one year of paid experience assessing risk and safety to vulnerable adults or children, AND one of the following:

One year as a Social Service Specialist 2;
OR
A Bachelor's degree in social services, human services, behavioral sciences or an allied field, and three years of paid social service experience performing functions equivalent to a Social Service Specialist 2;
OR
A Master's degree in social services, human services, behavioral sciences or an allied field and two years of paid social service experience equivalent to a Social Service Specialist 2. A two year Master's degree in one of the above fields that included a practicum will be substituted for one year of paid social service experience.
